If you are attending camp this year, please download the Camper Health History Form and Important Camp Notices available at the bottom of this page.
The Camper Health History Form comes from the state of Massachusetts, and we legally cannot keep any girl at camp without that form completely filled out along with a copy of a current physical form within the last two years. If you are worried about trying to schedule a doctor's appointment to get a physical, both Urgent Care and Minute Clinic offer walk-in summer camp physicals. Either is just as acceptable as a doctor's visit.
If this is your first year coming to camp, you will also need to fill out a photo release form, which can also be downloaded at the bottom of this page. Returning campers have their photo form stored at camp and do not need to fill one out again.
On registration day of the first day of your camp session, please bring with you the fully filled out Camper Health History Form, a copy of the camper’s most recent physical, a copy of up-to-date immunizations, a copy of your insurance card, any medication for the week in it's original container, and a photo release form if applicable. Any camper who shows up without these requirements may have her entry into camp delayed or denied.
With the application there is a packing list for your camper. Please go over it well in advance to ensure your camper has all the necessary items for a week away from home.
